Nurse Navigator - Stroke Program

Job DescriptionInova Fairfax Stroke Program is hiring for a Nurse Navigator RN for their Stroke Program.
Ideal candidate would have their stroke certificationThis Nurse Navigator guides stroke patients and family members through the health system from diagnosis, testing, treatment and follow-up care to assist with navigating the continuum of care.
Eliminates barriers to patient's access to health care services and facilitates continuity of care/care coordination.
Acts as a liaison, advocate and point of contact to patient by helping them achieve the best healthcare outcomes from doctors, hospitals and others who provide their required services.
May assist patients and families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources.
Acts as adviser/educator by providing emotional support, counseling, clinical education, resources and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
Builds working relationships with patient, healthcare providers, and community resources to maximize patient outcome and identify/resolve barriers to the plan of care.
Consistently seeks to improve healthcare experience and positive patient outcome.
Requirements
Experience:
Professional registered nurse with specific clinical knowledge and experience related to stroke or Neurosciences.
Minimum of 3 to 5 years progressive clinical nursing.
Education:
Graduate of an accredited school of Nursing; minimum BSN degree.
Certification:
Minimum of current Basic Life Support (BLS); SCRN certification within one year of hire or transfer into the job.
Licensure:
License or eligible for licensing in the Commonwealth of Virginia (RN).

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
