Community Outreach Worker - Peer Specialist
Job Description As a Community Outreach Worker Peer Specialist, you will provide support services for Fairfax Hospital's violence intervention program, dedicated to reducing, re-injuring and mortality rates among members of the community by providing intentionally injured patients with support during their stay in the hospital and beyond.

Responsibilities:
Under the direction of the Manager, you will provide hospital and community-based services to patients who have experienced intentional violent injury through assessment, crisis intervention, and counseling Will assist with hospital and community providers to participate in connecting and referring patients to appropriate services.
Serves as a positive role model to intentionally injured patients.
Maintain resource information to provide to connections.
Offer emotional support to patients who have experienced similar traumas.
Will provide accompaniment for victims of violent crime within the hospital, criminal justice system, and other systems as needed.
Qualifications:
Experience:
Zero (0) - Two (2) years' experience as a Community Health Educator working in the community or clinical setting; knowledge of community resources and patient engagement or Two (2) years' experience in lieu of degree/certification.
Education:
Associates Degree/Certification in pertinent field.
